What is vitamin D and why is it so important

Vitamin D isn't technically a vitamin. It's a steroid prohormone with receptors on more than 200 cell types. It regulates everything from gene expression to innate immunity.

  • Bone health: facilitates absorption of calcium and phosphorus in the gut. Without vitamin D, you can take all the calcium in the world and your bones won't use it.

The British paradox: abundant sunshine, scarce vitamin D

Britain spans from approximately 50°N (London) to 58°N (northern Scotland). It receives sufficient UVB radiation for vitamin D synthesis from April to September in southern regions, and only June to August in the north.

And yet, the prevalence of deficiency is similar to that of Scandinavia or Canada.

Why?

1. We live indoors

Cutaneous synthesis of vitamin D requires direct exposure of bare skin to UVB rays (280–315 nm). 15–20 minutes daily of arms and legs without protection would be enough to achieve 1,000 IU in fair skin (Fitzpatrick types I–II).

But the typical British working day:

  • 8h at home (no UVB, windows block >90%)
  • Commute by car/train (no UVB)
  • 8–10h in office (no UVB)
  • Journey back home (late afternoon, minimal UVB)
  • Indoor leisure (gym, cinema, sofa)

Result: actual sun exposure close to zero, even in the south in July.

2. Sunscreen and culture of sun fear

The dermatology industry has done excellent work preventing skin cancer. But the message has sunk so deeply that we apply SPF50 for a 10-minute trip to the shops.

An SPF30 blocks 97% of UVB. An SPF50 blocks 98%. If you apply it properly (2 mg/cm², reapplied every 2 hours), vitamin D synthesis is almost nil.

Context: melanoma represents <1% of skin cancers, and most occurs in areas not chronically exposed (torso, not face/arms). Intermittent intense exposure (sunburn) is the risk factor, not moderate regular exposure.

We're not saying you should burn. We're saying that 15 minutes of sun on arms and legs, without cream, mid-morning, 3–4 times per week, doesn't increase your melanoma risk. And it does normalise your vitamin D.

3. Latitude and solar angle

In winter, even in London (51°N), the solar angle is so low that UVB radiation is insufficient from November to February. You could stand naked in the sun for 2 hours and synthesise zero vitamin D.

Practical rule: if your shadow is longer than your height, synthesis is marginal.

4. Skin pigmentation

Melanin acts as a natural sunscreen. Dark skin (Fitzpatrick types IV–VI) needs 3–6 times more exposure than fair skin to synthesise the same amount.

In Britain, with growing immigration from South Asia, Africa and the Caribbean, the percentage of population at risk increases. And almost nobody supplements.

5. Age

The ability to synthesise vitamin D in the skin declines with age. By age 70, synthesis is 75% lower than at age 20, even with identical exposure.

If we add that people over 65 go out less, wear more clothing, and have less varied diets, the perfect storm is complete.

Optimal vitamin D levels: what the science says

The debate about optimal levels is Byzantine. The "normal" laboratory ranges (20–50 ng/ml) are based on avoiding rickets or osteomalacia, not on optimising immune, muscular or cognitive function.

Clinical classification (serum 25-OH vitamin D):

  • Severe deficiency: <10 ng/ml (rickets, osteomalacia)
  • Deficiency: 10–20 ng/ml (secondary hyperparathyroidism, bone loss)
  • Insufficiency: 20–30 ng/ml (low "normal" range)
  • Sufficiency: 30–50 ng/ml
  • Optimal (functional medicine): 40–60 ng/ml
  • Excess: >100 ng/ml (hypercalcaemia, toxicity)

Recent meta-analyses in immunity show that maximum benefits in preventing respiratory infections are seen in ranges ≥40 ng/ml.

Studies in longevity and mortality (prospective cohorts) show a U-shaped curve with minimum risk between 40–60 ng/ml. Below that, cardiovascular mortality increases. Above 60, no additional benefit and some studies suggest risk (though causality is unclear).

Symptoms of vitamin D deficiency (spoiler: they're vague)

Mild deficiency is asymptomatic. Moderate deficiency is insidious. There's no red alarm. Just slow deterioration.

Signs that should prompt you to get tested:

  • Chronic fatigue with no obvious cause
  • Muscle pain or weakness (especially in thighs, calves)
  • Frequent respiratory infections (>3 per year)
  • Low mood, especially in winter (seasonal pattern)
  • Bone fragility or fractures from minor trauma
  • Slow wound healing
  • More hair loss than usual

In children: stunted growth, bone deformities (bowed legs), irritability.

In people over 65: frequent falls (muscle weakness), hip fractures.

The only way to know is to test serum 25-OH vitamin D. It costs £15–25 in private laboratories without a prescription. On the NHS, it's only requested if there's clinical suspicion.

Food sources of vitamin D: insufficient

Vitamin D in food is marginal. Nature expects you to synthesise it in your skin.

Content in common foods (IU per serving):

  • Wild salmon (100g): 600–1,000 IU
  • Farmed salmon (100g): 100–250 IU
  • Tinned tuna (100g): 150–200 IU
  • Beef liver (100g): 50 IU
  • Egg yolk (1 unit): 40 IU

To reach 2,000 IU daily from diet alone, you'd need to eat wild salmon every day. Or 50 eggs. Impractical.

The traditional British diet is poor in vitamin D. Oily fish once or twice weekly doesn't cover the deficit if you're not getting sun exposure.

Practical rule: every 1,000 IU daily raises serum levels approximately 5–10 ng/ml after 3 months (in people with normal weight and normal absorption).

Toxicity: virtually impossible with doses <10,000 IU/day long-term. Documented hypervitaminosis D occurs with prolonged doses >40,000 IU/day or manufacturing errors (rare cases). Symptoms (hypercalcaemia) are nausea, vomiting, extreme muscle weakness.

Essential cofactors:

  1. Vitamin K2 (MK-7): vitamin D increases osteocalcin synthesis, but you need K2 to activate it and direct calcium to bones (not arteries). Dose: 100–200 mcg/day.
  2. Magnesium: cofactor in the conversion of vitamin D to its active form (calcitriol). Magnesium deficiency (very common in Britain) limits effectiveness. Dose: 300–400 mg/day, preferably as magnesium glycinate.
  3. Fat in your meal: vitamin D is fat-soluble. Take it with food containing fat (avocado, nuts, olive oil, salmon, eggs).

Timing: immaterial. Some prefer morning (with fatty breakfast). Others with the main meal of the day.

The market is saturated with mediocre options. What to look for:

  • D3 form (cholecalciferol), not D2.
  • Oil-based format (softgel or drops), not tablets. Bioavailability is higher.

We don't want to demonise the sun. The sun is good. Excess and burns are not.

  • Without sunscreen during those 15–20 minutes. After, apply it.

FAQ: Frequently asked questions about vitamin D

Can I take vitamin D without testing first?

Yes, if you use conservative doses (2,000 IU/day). It's practically impossible to poison yourself with that dose. But testing is cheap and lets you personalise the dose to reach your target faster.

How long does it take to raise levels after starting to supplement?

8–12 weeks to see peak effect. That's why we recommend retesting at 3 months, not sooner.

Vitamin D2 or D3?

D3 (cholecalciferol), always. D2 is less effective and has a shorter half-life. D3 is the form your skin synthesises.

Can I take vitamin D at night?

Yes, but there are anecdotal reports that high evening doses (>5,000 IU) interfere with sleep in some people. If you notice that, switch to morning dosing. For most people, timing doesn't matter.

Is it better to take one mega-dose weekly or daily doses?
